Star Icon Program of- Jewish Family Service Association (JFSA)
A unique program designed to support individuals and caregivers. Taking care of a loved-one who has Alzheimer’s disease or other dementia-related disorders can cause stress and frustration. It is critical to have access to timely information and local resources, support in creating and managing a plan of care, and respite that allows you to take a break from your caregiving responsibilities. If a loved-one is showing signs of cognitive difficulties and you suspect the cause may be Alzheimer’s or another related disorder, call JFSA. In either situation, this program can help you find solutions. Our program is designed to provide support and find the supports to help you to identify the services you need.
HOW IT WORKS
You and your Care Navigator work together, along with a geriatrician and other professional staff, to develop a care plan that includes:
• Screening and assessment of your loved-one, if necessary
• Information and referrals to appropriate community services based on your loved-one’s needs, such as homecare, education about brain health and dementia, companion care, adult day support or assisted living
• Coordination of care – between medical, social services and other providers as well as communicating with families to provide support, education and peace of mind
CAREGIVING FOR SPECIAL NEEDS INDIVIDUALS
This program is also available to paid and unpaid caregivers of individuals with developmental disabilities who have been diagnosed with Alzheimer’s or other related disorders.
FEES
Screening and assessments, referrals, Care Navigator services are provided free of charge. Some referred services may have fees associated with them.
